High tolerance is the body and brain adapting to ongoing substance use — and it's both a sign of substantial use history and a risk factor for serious harm. Tolerance is the physiological adaptation that occurs when the brain and body are repeatedly exposed to a substance. The mechanisms vary by substance but generally involve:

Tolerance can develop with regular medical use of medications (opioid pain medications, benzodiazepines for anxiety) without addiction. What distinguishes tolerance plus addiction from tolerance alone is the broader pattern: loss of control over use, continued use despite harm, cravings, time spent obtaining and recovering from use, role obligations affected. High tolerance plus these other patterns is what indicates substance use disorder.
Yes — abstinence breaks reduce tolerance. But this approach is risky: many overdose deaths involve people returning to previous doses after tolerance drops. Depends on the broader picture. High tolerance alone isn't a treatment indication, but high tolerance combined with the other DSM-5 criteria for substance use disorder usually warrants clinical attention. The decision isn't "do I have to," it's "what would treatment provide and what's the cost of not getting it." A clinical assessment can sort out what level of care is appropriate.
Could be one or more of: more substantial use history than you've acknowledged, genetic factors in metabolism, body composition, age, gender, concurrent substance use, certain medical conditions. "Naturally high tolerance" without significant exposure history is rare; what often presents as high baseline tolerance is actually accumulated tolerance from use patterns that have been minimized.
